Preparing for instrument check ride?

The instrument checkride is the hardest you'll ever take. If you're "super nervous" about it, I'd say you're not ready for it yet and your instructor has not properly prepared you for the checkride. Your question about "some sort of checklist" and "like everything from documents needed to fly the plane to making a safe instrument landing" clearly indicates your level of unpreparedness.
